Program Description

TLP serves youth ages 16-21 years old who can no longer live at home, have no other safe living alternatives and are not in state custody. Case managers assist youth with long-term housing and the successful transition to independent living. TLP services include: long-term housing support; assistance in accessing area resources (e.g., job training, counseling, education and healthcare); case management; advocacy; life skills instruction; financial support; and emotional support.
