Programs: Youth in Transition

Program Description

The Youth in Transition Program is a youth-driven, family-guided program for young people ages 16-21 with severe emotional disturbance. The program focuses on helping youth prepare for adulthood by ensuring that they have all the supports they need.

The program:

  • Supports young adults in self-advocacy, leadership, and empowerment;
  • Engages young adults in making the decisions that affect their lives;
  • Improves the system of care for young adults; and
  • Creates meaningful, lasting relationships between young people and caring adults.


Engaging youth in 'systems of care' means creating opportunities for youth people to guide the systems that serve them. Partnership with youth is a core value of the systems of care philosophy. For that reason, the Youth in Transition actively seeks and welcomes youth involvement.
